This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment to your privacy
Swanlight Behavioral Health (the "practice") is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, follow the terms of the notice currently in effect, and notify affected individuals following a breach of unsecured PHI.
How we may use and disclose your health information
We may use and disclose your PHI for the following purposes without your written authorization:
Treatment. To provide, coordinate, or manage your healthcare and any related services with other providers involved in your care.
Payment. To bill and receive payment for the services we provide.
Healthcare operations. For internal operations such as quality assessment, training, and licensing activities.
Required by law. When use or disclosure is required by federal, state, or local law.
Public health and safety. To prevent or control disease, report abuse or neglect, or address serious threats to health or safety.
Health oversight, judicial proceedings, law enforcement, and similar purposes as permitted by law.
Special protections for substance use disorder records (42 CFR Part 2)
Federal law (42 CFR Part 2) provides additional confidentiality protection for substance use disorder (SUD) treatment records that we receive, create, or maintain. SUD records may not be redisclosed without your written consent except in specific circumstances permitted by law. When we share SUD records with another party with your consent, we include a notice prohibiting redisclosure as required by 42 CFR Part 2.
Uses and disclosures that require your written authorization
The following uses and disclosures require your written authorization:
Most uses and disclosures of psychotherapy notes (where applicable).
Marketing communications.
Sale of PHI.
Most uses and disclosures of substance use disorder records protected by 42 CFR Part 2.
Other uses and disclosures not described in this notice.
You may revoke your authorization in writing at any time, except to the extent we have already acted in reliance on it.
Your rights regarding your health information
Right to access. You may inspect and obtain a copy of your PHI.
Right to amend. You may request corrections to your PHI.
Right to an accounting of disclosures. You may request a list of certain disclosures we have made.
Right to request restrictions. You may request limits on certain uses and disclosures.
Right to confidential communications. You may request that we contact you in a specific way or at a specific location.
Right to a paper copy of this notice. You may request a paper copy at any time.
Right to be notified of a breach. You will be notified if a breach of unsecured PHI affects you.
Right to opt out of certain communications. You may opt out of fundraising communications, where applicable.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
